3. Follow Prompts: ‘For myself’ then ‘Existing Patient’
4. Select: ‘Maximum Health Pathology Blood Test’ & continue
5. ’Choose a time’: note this is not an in-person appointment time-this action creates a Request to me & I will email your Blood Test Form to you within 24 hours of this time - usually sooner. 
6. **Please message me your email address for me to send you the Bloods form**
7. You may need to check your JUNK email folder.
